Job Description:
A company is looking for a Social Worker Ambulatory HOPS.
Key Responsibilities
Assist patients and families in achieving improved health outcomes
by addressing obstacles to health resource utilization
Assess psychosocial risk factors and develop care plans based on
patient and community needs
Collaborate with healthcare providers for care coordination and
transition management
Required Qualifications, Training, and Education
Master's degree in social work (MSW)
Current state licensure as a Social Worker (LSW, LISW, LISW-S, or
PC)
Experience in ambulatory care, hospital patient care, or a related
field
Knowledge of community resources
Basic Life Support (BLS) certification through American Heart
Association (AHA)
